Schedule: Tuesday - Saturday; 3:00 pm - 11:00 pm or Sunday - Thursday; 3:00 pm - 11:00 pm   (This position requires a vehicle)   Trilogy’s Residential Program provides group counseling, training, and assistance with independent living skills, medication monitoring, and opportunities for social participation.    The Assistant Team Leader is responsible for assisting Team Leader/Program Manager in managing the day-to-day functions of the team.  These duties will vary depending on the needs of the team, but may consist of reviewing paperwork and documentation, team scheduling, providing consultation, training, crisis management, coaching, mentoring team members, and covering sites etc.  The Assistant Team Leader will also maintain a reduced caseload and perform daily duties of a House Manager/Recovery Counselor. They will assist residential department in providing continuous support to residential clients in order to help them achieve their recovery goals and develop skills to transition to more independent living.  The Assistant Team Leader will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

  $26.44 - 28.84 7:00 am - 7:00 pm; 3 days per week + 4 Hour Admin Shift Full days: Monday and Tuesday; alternating Wednesday / Saturday   Trilogy’s First Response Alternative Crisis Team (FACT) provides mobile crisis response services to community members experiencing mental health crises or distress. Available to all community members free of charge, the FACT team engages with a wide population of individuals experiencing suicidal ideation, psychosis, housing instability, severe anxiety, and other time sensitive needs. FACT Crisis Counselors support with the creation of safety plans, connection to community resources, and in the moment crisis counselling.   The Senior Crisis Counselor responds to mental health crises in the community as part of a team and provide de-escalation, assessment, safety planning, and community resources.  They will provide strengths-based, person-centered, and trauma-informed services to people with severe mental illnesses and co-occurring substance abuse utilizing a harm reduction approach.   The Senior Crisis Counselor is a member of the FACT leadership team along with the Team Leaders, Program Manager, and Clinical Director. While the Senior Crisis Counselor does not supervise staff members, they write the crisis line phone schedule, write the daily appointment schedule, assign tasks to staff, and answer questions from staff throughout the day. The Senior Crisis Counselor is responsible for managing dispatch and workflow of tasks, based on clinical urgency and staffing. The Senior Crisis Counselor will compile data for monthly reports, and will assist with onboarding new employees.   This role works three 12-hour in person shifts during the week, and 4 hours remotely each week. The in-person shifts are 6:30am-7pm pm Monday, Tuesday, alternating Wednesdays, alternating Saturdays. This position is required to work some holidays. There is no on-call responsibility for this position.

Schedule: Monday - Friday; 8:00 am - 4:30 pm   Trilogy is considered a Prime organization under the Comprehensive Class Member Program and provides support services to Williams and Colbert Class Members participating in the program.  The Williams and Colbert Consent Decrees are two of the largest Olmstead settlements in Illinois, with over 24,000 Class Members. Through the settlements, the State of Illinois agreed to provide supports and services to help Class Members transition from institutional settings to live in the community.  Class Members can live in the most integrated setting that meets their needs.   The Transition Team Leader oversees the Transition Coordination team that is responsible for supporting the Comprehensive Class Member Transition Program in achieving program goals.  Theywill provide direct clinical supervision of team members and manage employee workflow ensuring the transition process from nursing home to independent living in the community for class members.  They will runteam meetings, help develop and monitor treatment plans for the team’s clients, anddevelop and administer tracking systems for required documentation to ensure quality service and effective intervention are delivered to Trilogy clients. The Team Leader will routinely interface with other supervisory employees to enhance communication, cooperation/collaboration across programs/departments, and to improve the quality and comprehensiveness of services provided.  The Transition Team Leader will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

This position is hybrid, with the outpatient clinic located in Albany Park in Chicago.   This position will also provide community-based case management services. This position is eligible for a condensed work schedule.
