As a Registered Nurse on the Colbert team you will have the opportunity to make a difference in the lives of the people we serve each and every day by helping them discover their own unique self, purpose, community and wellness. Trilogy fully embraces a collaborative team approach to each client’s treatment in order to enhance our clients’ recovery from severe and persistent mental illness.
The Colbert Registered Nurse will provide direct patient care through educating Colbert Class members on medical illness self management, providing medication monitoring and training and medication reconciliation services, educating the behavioral health team on Class Member’s medical issues and providing care coordination services by communicating directly with physicians, specialists, pharmacies, home health providers and inpatient/outpatient providers. In this position you will make preliminary assessment of clients’ health problems while they are still residing in the nursing home and will develop medical treatment plan goals for the client to help them transition into an independent community setting. You will also collaborate with clinical and non-clinical staff on patient plans of care and ensure quality standards are met. You will spend a significant amount of time in the community visiting clients in their homes.
Trilogy strongly believes that treatment services are more effective when provided in the client’s natural setting, hence, the majority of services are provided in the community. Trilogy also employs a holistic approach to each client’s treatment and integrates all facets of our clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.
Candidates are required to have a Registered Nurse license in the state of Illinois. The ideal candidate will have a minimum of one year of nursing experience preferably in a community mental health setting. This full-time position will require some flexibility with hours and may include some weekend hours, as needed.
