    Trilogy Behavioral Healthcare Inc. strongly believes that treatment services are more effective when provided in the client’s natural setting, hence, the majority of services provided are located in the community.  Trilogy Behavioral Healthcare Inc. also employs a holistic approach to each client’s treatment and integrates all facets of our clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.

     

    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    Assists African Americans who are experiencing weight-related health concerns and have a serious mental illness with obtaining healthy eating and increased physical activity goals, in group and individual settings.  Utilizes the Behaviors for Healthy Lifestyles manual and workbook material to discuss and engage participants in developing better eating habits and engaging in physical activity.  Accompanies participants in pursuing healthy eating and exercise goals in the community (e.g. signing up for gym membership, shopping for fruits and vegetables). Utilizes and discusses personal experiences regarding his or her struggles with health goals and mental illness. Engages participants by compassionately and collaboratively assessing participant needs and goals, addressing barriers to wellness, providing linkage to services and resources, and developing plans to achieve wellness goals.

    Essential Duties & Responsibilities:

    • Engages with each participant within the community where the participant lives and works; at least 50% of time is spent in the community.
    • Travels with individual participants or groups of participants in the community to engage in healthy activities (e.g. exercising, grocery shopping, cooking skills)
    • Leads group discussions and workbook engagement exercises.
    • Leads group exercise classes.
    • Assists group members in completing a weekly goal setting activity.
    • Collaborates with participants in creating a plan to address the participant’s physical health and wellness goals.
    • Compiles and shares healthy living resources in participant communities.
    • Works with participants through peer support and frequent communication, utilizing relationship building, selective self-disclosure, motivational interviewing, and a strength-based approach.
    • Completes reports as required, including contact logs and case summarizes.
    • Participates in meetings, conferences, and committees as assigned.
    • Communicates regularly with team and supervisor.
    • Maintains a caseload of participants.
    • Performs other duties as assigned.

    • Education and/or Experience:
      • High school diploma (Associate’s Degree preferred in a health and human services related field).
      • Experience with and appreciation of the health and human service needs of vulnerable populations.
      • Life experience similar to that of members of the population with which he/she is assigned to work.
      • Experience with recovery from mental illness.
      • Personal experience with weight-related health challenges and with navigating exercise and healthy-living resources in the community.
    •  Skills/Abilities:
      • Ability to engage individuals who are experiencing mental illness and substance use.
      • Ability to work with vulnerable populations in a non-judgmental manner and to work as a member of a multi-disciplinary team.
      • Aware of his/her life experience and its impact on his/her health care participation and physical health goals and is able to share this experience with participants served to help participants meet their own goals.
      • Ability to communicate effectively both orally and in writing; basic computer skills.
      • Must be able to multi-task, have initiative, and be self-directed. Ability to endure periods of heavy workload or stress.
      • Ability to work with frequent interruptions and respond appropriately to unexpected situations.
      • Ability to transport self to and from off-site locations.
      • Ability to work with other members of the healthcare team and community to provide quality services.

      • Frequently required to stand, sit, walk, and commute.
      • Ability to lead low impact, moderate exercise classes, up to three times per week.

      • Daily access to a well maintained vehicle with 100,000/300,000 liability insurance
