    Trilogy Behavioral Healthcare Inc. strongly believes that treatment services are more effective when provided in the client’s natural setting, hence, the majority of services provided are located in the community.  Trilogy Behavioral Healthcare Inc. also employs a holistic approach to each client’s treatment and integrates all facets of our clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.

     

    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    Assists African Americans who are experiencing weight-related health concerns and have a serious mental illness with healthy eating and increased physical activity goals in group and individual settings.  Utilizes the Behaviors for Healthy Lifestyles manual and workbook material to discuss and engage participants in developing better eating habits and engaging in physical activity. The facilitator is expected to have an understanding of mental illness and substance use and be willing to engage individuals who are experiencing these.  The facilitator must have experience with leading group meetings and the ability to engage participants in low impact and moderate intensity physical activity.   

    • Engage the individual in service delivery
    • Assist individuals in understanding the written material
    • Promote understanding of the individual's needs within the parameters of healthy eating habits and increased physical activity
    • Help the individual incorporate newly learned information into his or her daily life
    • Advocate for the individual and help reduce barriers to change 

    Essential Duties and Responsibilities:

    • Lead group discussions and workbook engagement exercises.
    • Lead group exercise classes.
    • Assist group members in completing a weekly goal setting activity.
    • Perform monthly individual 20-minute session with each participant to review the activity tracking worksheets.
    • Accurately completes reports as required. Reports may include daily encounter logs, data monthly submissions and summaries of class attendance and engagement strategies.  
    • Participates in meetings, conferences and committees as assigned.
    • Other duties as assigned.

    • Education and/or Experience:
      • High school diploma.
      • Demonstrated experience with leading a group discussion and appreciation of the health and human service needs of vulnerable populations.
      • Ability to work with vulnerable populations in a non-judgmental manner and has the ability to work as a member of a multi-disciplinary team.
    •  Skills/Abilities:
      • Ability to communicate effectively both orally and written, and have basic computer skills.
      • Must be able to multi-task, have initiative, and be self-directed. 
      • Ability to endure periods of heavy workload or stress. 
      • Ability to work with frequent interruptions and respond appropriately to unexpected situations. 
      • Ability to transport self to and from off-site locations.
      • Ability to work with other members of the healthcare team and community to provide quality health care services.

      • Frequently required to stand, sit, walk, and commute.
      • Ability to lead low impact, moderate exercise classes, up to three times per week.

      • The noise level in the work environment is usually moderate.

      • Valid IL Driver’s License
      • Daily access to a well maintained vehicle with 100,000/300,000 liability insurance
