The Community Health Nurse will support Williams and Colbert Class Members in the transition process from institutional living to independent community living. The Community Health Nurse will support Williams and Colbert Class Members and clients on Community Support Teams (CST) to maintain their health while living in community settings. They will provide supervision and consultation on medical illness, self-management, and medication monitoring, training, and medication reconciliation services. They will collaborate with the treatment team, providers, and others on client plans of care to ensure quality standards are met. The Community Health Nurse will provide strengths-based, client-centered, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disordersutilizing a harm reduction approach
The nursing duties will be coordinated with the pre-transition treatment team, CST teams, Providers, and others involved in the client’s plan of care to ensure quality standards are met. They will ensure reconciliation of medications and care as they transition into the community, attend doctor visits or any movement in and out of the facilities to ensure quality care is sustained.

The Health Mentor is responsible for working with clients enrolled in the InSHAPEprogram to develop and implement their personal wellness plan. They will provide support, education, and motivation related to physical fitness, nutrition, and wellnessto clients in the community. The Health Mentor must have a personal trainer certification or must obtain a personal trainer certification within 6 months of being hired.
The Assistant Team Leader is responsible for assisting the Team Leader in managing the day to day functions of the team. They will provide individually based motivational treatment and support to clients to assist them in their recovery from mental illness. Theywill be an advocate for clients and link them to community services as needed.
The RSS will be SOAR trained andassist in navigating the system of gathering relevant dataand working with clients to obtain benefitswhile building a therapeutic relationship and remaining on top of deadlines. They will provide assertive outreach, obtain client’s medical records, manage communication with the Social Security Administration (SSA) and Determination Services (DDS), provide a detailed narrative, and obtain a co-signature from medical providers. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.
The RSS must complete online training for SOAR and successfully receive their SOAR certification within 90 days of hire and will provide at least 60-75% of all community support services in the community. The Housing Coordinator will identify suitable housing for Comprehensive Class Member Transition Program members. They will collaborate with Transitional Coordinators and with representatives from the city, state, and county to secure safe and affordable housing to fit the Class Member’s specific needs. They will also work with the interdisciplinary team to identify Class Member preferences, background, and accessibility needs to support in locating secure housing in an efficient manner. The Housing Coordinator will establish and maintain relationships with rental owners, landlords, and/or property managers while maintaining a housing resource list. They will also create housing marketing and educational materials that reflect the mission of Trilogy and the Comprehensive Class Member Transition Program goals.
