    Trilogy Behavioral Healthcare Inc. strongly believes that treatment services are more effective when provided in the client’s natural setting, hence, the majority of services provided are located in the community.  Trilogy Behavioral Healthcare Inc. also employs a holistic approach to each client’s treatment and integrates all facets of our clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.

     

    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    The Care Coordinator will be Trilogy’s clinical liaison to managed care organizations (MCOs) and ensures compliance with HEDIS measures.  Care Coordinator will work directly with contracted MCOs to find, engage, assess, and connect members to medically necessary care.  Metrics defined by Trilogy and MCOs will be monitored and reported on.  Care Coordinator works with departments and programs across the organization to ensure care coordination standards are met.

     

    Essential Responsibilities

    • Prioritize potential clients in need of community based care coordination with MCOs with whom we have a care coordination contract.
    • Find those disconnected from MCOs and treatment through review of claims (when available) and initiate contact.
    • Engage and advocate for clients, linking them to needed community services. Assess client’s personal, medical, social, emotional, and environmental situation and make appropriate referrals to care.
    • Meet deliverables based on MCO care coordination contracts and complete required reporting
    • Organize and direct care coordination efforts across other teams and programs, as needed.
    • Develop, implementation, and track HEDIS measures across the organization.
    • Serve as liaison between MCOs and Trilogy.
    • Maintain prior authorization, documentation, and charting in compliance with MCOs, Medicaid, state specific accreditation, and agency standards.
    • Supervise, train, and assign duties to staff, students, and interns as needed.
    • Perform other job-related duties and/or projects as assigned.

    Qualifications

    • Masters’ Degree in Social Work, Psychology, or Counseling required
    • Minimum five years’ experience working with people with mental health and/or substance abuse diagnoses in a leadership role
    • Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, or Licensed Practitioner of the Healing Arts preferred
    • Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
