    Trilogy Behavioral Healthcare Inc. strongly believes that treatment services are more effective when provided in the client’s natural setting, hence, the majority of services provided are located in the community.  Trilogy Behavioral Healthcare Inc. also employs a holistic approach to each client’s treatment and integrates all facets of our clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.


     


    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.


     


    Heartland Health Centers is contracting with Trilogy to provide an LCSW/LCPC at their school-based clinic at Chicago Public Elementary Schools.  This position will be located at the school for 10 months out of the year providing a variety of specialized services to the medically underserved, chronically mentally ill, or other special populations.  The School-based Social Worker will be off during the summer months.


     

    Responsibilities

    Essential Responsibilities: 



    • Assists primary health care providers in recognizing and treating mental disorders and psychological problems.

    • Coordinates/collaborates with health center staff on potential patients to be referred to therapy or case management.

    • Provides advocacy, crisis intervention, counseling and problem solving in a variety of areas for patients. Works with our integrated health team.

    • Consults with psychiatrists and medical providers regarding patient treatment.

    • Work as part of an interdisciplinary team to provide patient care and coordination.

    • Presents clinical case reviews.

    • Develops implements, oversees and follows-up with patient treatment plans to ensure that realistic goals and deadlines of accomplishments are being met.

    • Establishes and maintains a professional and therapeutic relationship within the standards of patient profiles, confidentiality laws and the newest clinical research available.

    • Maintains documentation of patient’s individual and group treatment, by using current and accurate ICD – Coding and CPT coding.

    • Assimilates the knowledge of resources available to implement the clinical concepts.

    • Develops and maintains relationships with outside providers, case managers and hospitals.

    • Participates in evaluation of peers and support staff.

    • Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.

    • Assists in preventing relapse or morbidity in conditions that tend to recur over time.

    • Evaluates patient care plans with primary care team.

    • Teaches patients, families and staff about care, prevention and treatment enhancement techniques.

    • Monitors the site’s behavioral health program, identifying problems related to patient services and making recommendations for improvement)

    • Adheres to professional standards as outlined by governmental bodies, NASW and/or other appropriate professional associations, private funding sources, organization plans/policies and organization’s guidelines.

    • Participates in periodic evaluative reviews and/or in-house and external staff training.

    Qualifications

    Requirements:



    • Licensed in Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC).

    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

    • Ability to write routine reports and correspondence.

    • Ability to speak effectively before groups of customers or employees of organization.

    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

    • Ability to deal with problems involving several concrete variables in standardized situations.
