    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    The Program Manager will oversee the Outpatient Program at Trilogy’s Peterson location, and has overall responsibility for the direct supervision of a team of clinical staff. They will provide direct clinical supervision for team members, run team meetings, monitor treatment plans for the team’s clients, and administer tracking systems for required documentation to ensure quality service delivery and effective interventions to clients. The Program Manger will deliver individual, group, and family clinical services to clients. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

     

    Essential Responsibilities

    • Provide direct service to clients and their families, functioning as the Senior Clinician for the Outpatient program.
    • Screen all intake calls. Adjust employee schedules and allocate resources accordingly to ensure enough clinical time is scheduled to meet direct service requirements.
    • Provide effective supervision and coaching to all assigned team members. Train new employees to provide quality services to clients.  Ensure continued education and training for assigned clinical and administrative staff to strengthen services provided.
    • Obtain program referrals and build relationships to develop and promote the program.
    • Recommend modifications in service structure or operations that affect the program. Develop plans to improve program performance.
    • Provide comprehensive bio-psychosocial assessment services to clients and their families.
    • Develop individualized treatment plans with the input of the client and their team, which includes providers and natural supports. Collaborate with members of the client’s team to ensure quality of treatment and coordination of care.
    • Maintain documentation and chart in compliance with Medicaid, Commission on Accreditation of Rehabilitation Services (CARF), and agency standards.
    • Participate in meetings, committees, and work groups as needed.
    • Perform other related duties and/or projects as assigned.

     

    Qualifications

    This position requires a minimum of ONE of the following:

    • LCSW (Licensed Clinical Social Worker) with a valid Illinois License
    • LCPC (Licensed Clinical Professional Counselor) with a valid Illinois License

     

    Other Requirements include:

    • Valid IL Driver’s License
    • Daily access to a well maintained vehicle with 100,000/300,000 liability insurance
    • Minimum 2 years experience in mental health settings
    • Supervisory experience highly preferred
