    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    The Resident Reviewer will work with Colbert Class Members in nursing home facilities to provide assessment services to residents and serve as an advocate to residents to help facilitate a positive transition from nursing home living to independent living in the community. He/she will assess the residents’ personal, medical, emotional, social, and environmental situation to plan for linkage and treatment course. The Resident Reviewer will routinely interface with nursing home staff to obtain resident’s bio-psychosocial historical data. He/she will collaborate with transitional teams to report on their assessment experience and provide counsel as the team works on transitioning the resident into the community.

     

    The Resident Reviewer will provide at least 80% of resident review services in nursing home facilities.

     

     

    Essential Responsibilities

    • Engage Colbert Class Members in designated nursing home facilities to obtain consent to conduct resident review assessments
    • Conduct face-to-face interview assessments (initial contact, annual review, or as requested) with members to determine if member would be appropriate to transition into independent living.
    • Complete all resident review assessments using state tool, and submit to appropriate agencies; e.g. The Illinois Department of Aging
    • Track, monitor, and report on metrics and outcomes

    • Provide effective supervision and coaching to all assigned team members. Train new employees to provide quality services to clients. Ensure continued education and training for assigned clinical and administrative staff to strengthen services provided.
    • Perform other related duties and/or projects as assigned

       

       

    Qualifications

    This position requires a minimum of ONE of the following:

    • LCSW (Licensed Clinical Social Worker) with a valid Illinois License
    • LCPC (Licensed Clinical Professional Counselor) with a valid Illinois License
    • RN (Registered Nurse) with a valid Illinois License

     

    Other Requirements include:

    • Valid IL Driver’s License
    • Daily access to a well maintained vehicle with 100,000/300,000 liability insurance
    • Minimum two years’ experience working with people with mental health and/or substance abuse diagnoses
    • Experience working with nursing homes highly preferred
    • Exceptional customer service skills and ability to interact with a diverse group of internal/external stakeholders
    • Supervisory experience preferred
