    Typical services provided include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

    Responsibilities

    The Assistant Team Leader has overall responsibility for the direct supervision of House Managers at the Crandon Residential site. He/she will provide direct clinical supervision for team members, run regularly scheduled team meetings, monitor/manage the monthly work schedule with assistance from the Residential Program Manager, and administer tracking systems for required documentation. The Assistant Team Leader will also provide onsite/offsite House Manager services to clients during assigned shifts.

     

    Essential Responsibilities

    • Work directly with clients during assigned shifts to address client issues regarding case management needs, training in independent living skills (i.e. personal hygiene, housekeeping skills, nutrition, etc.), provide individual services and group services, or crisis intervention.Provide quality individual or group services to clients at residential programming sites primarily performed onsite, but can be community-based; i.e. medical appointments, grocery store visits, etc.
    • Plan and facilitate special activities and outings for the residents.
    • Maintain documentation and chart in compliance with Medicaid, Commission on Accreditation of Rehabilitation Services (CARF), and agency standards
    • Provide effective supervision and coaching to all assigned team members. Train new employees to provide quality services to clients. Ensure continued education and training for assigned clinical and administrative staff to strengthen services provided.
    • Evaluate program’s effectiveness in meeting deliverables. Recommend modifications in service structure or operations that affect the program. Develop plans to improve program performance.
    • Collaborate with team members and outreach treatment teams to deliver quality service and effective interventions on behalf of the clients. Ensure the team provides client-centered and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorder.
    • Partner with transition teams, Occupational Therapy, and Supervisor to review and admit clients into the Residential program. Oversee the safe transition of the client into the community.
    • Provide administrative support by assisting in the completion and/or review of required assessments (i.e. mental health assessments, individualized recovery plans, etc.) and insurance authorizations
    • Oversee emergency drill processes and inspections. Ensure drills and inspections are conducted timely.
    • Perform other related duties and/or projects as assigned

    Qualifications

    This position requires ONE of the following:

    • A licensed social worker (LSW) possessing at least a master's degree in social work with specialized training in mental health services or with at least 2 years experience in mental health services;
    • A licensed professional counselor (LPC) possessing at least a master's degree with specialized training in mental health services or with at least 2 years experience in mental health services;

    Other Requirements include:

    • Valid IL Driver’s License
    • Daily access to a well maintained vehicle with 100,000/300,000 liability insurance
    • Experience working with people with mental health and/or substance abuse diagnosis preferred
