The Program Manager will oversee Trilogy’s South Chicago Residential Sites. The Program Manager will be responsible for the general welfare of the clients and employees of their assigned treatment teams.  They will provide direct clinical supervision, ensure the team meetings run effectively, monitor the quality and efficiency of all program/team services provided to clients and develop and implement tracking systems for required documentation. They will be responsible for the development, operation, administration, and evaluation of their program/teams including CCMTP clients and processes. They will have working knowledge of the Coordinated Entry System and CCMTP process for clients moving into the residential programs and ongoing requirements. The Community Health Nurse will support Williams and Colbert Class Members in the transition process from institutional living to independent community living. The Community Health Nurse will support Williams and Colbert Class Members and clients on Community Support Teams (CST) to maintain their health while living in community settings.  They will provide supervision and consultation on medical illness, self-management, and medication monitoring, training, and medication reconciliation services.  They will collaborate with the treatment team, providers, and others on client plans of care to ensure quality standards are met.  The Community Health Nurse will provide strengths-based, client-centered, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disordersutilizing a harm reduction approach     The nursing duties will be coordinated with the pre-transition treatment team, CST teams, Providers, and others involved in the client’s plan of care to ensure quality standards are met.  The Community Health Nurse will work with clients directly in the community (50-70%) engaging them in their home settings.  They will ensure reconciliation of medications and care as they transition into the community, attend doctor visits or any movement in and out of the facilities to ensure quality care is sustained.

Schedule: Monday - Friday; 8:00 am - 4:30 pm   Trilogy is considered a Prime organization under the Comprehensive Class Member Program and provides support services to Williams and Colbert Class Members participating in the program.  The Williams and Colbert Consent Decrees are two of the largest Olmstead settlements in Illinois, with over 24,000 Class Members. Through the settlements, the State of Illinois agreed to provide supports and services to help Class Members transition from institutional settings to live in the community.  Class Members can live in the most integrated setting that meets their needs.   The Transition Team Leader oversees the Transition Coordination team that is responsible for supporting the Comprehensive Class Member Transition Program in achieving program goals.  Theywill provide direct clinical supervision of team members and manage employee workflow ensuring the transition process from nursing home to independent living in the community for class members.  They will runteam meetings, help develop and monitor treatment plans for the team’s clients, anddevelop and administer tracking systems for required documentation to ensure quality service and effective intervention are delivered to Trilogy clients. The Team Leader will routinely interface with other supervisory employees to enhance communication, cooperation/collaboration across programs/departments, and to improve the quality and comprehensiveness of services provided.  The Transition Team Leader will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.
