Typical services include case management, counseling/therapy, individual skills training, medication training, crisis intervention, linkage to community resources, advocacy, assessment, and treatment planning.

Responsibilities

Trilogy Behavioral Healthcare is seeking a skilled, dynamic, and self-motivated clinician to serve as a part-time (32 hours/week) School-Based Therapist. The School-Based Therapist will provide individual and family services to students at one of four CPS schools located in the West Ridge and Logan Square neighborhoods.  He/she will provide strengths-based, client-centered, and trauma-informed services to clients with mental health diagnoses and co-occurring substance abuse disorders.  Services will include an emphasis on meeting behavioral health needs in support of school functioning and achievement. The School-Based Therapist will develop relationships with behavioral health staff and other school personnel, will participate in relevant meetings and groups at each of the CPS schools, and will provide consultation to school faculty and staff as needed.

 

Essential Responsibilities:

  • Receive referrals directly from school social workers, behavioral health teams, and other school personnel.
  • Provide comprehensive bio-psychosocial assessment services to children, adolescents, and their families
  • Develop individualized treatment plans with the input of the client and their team, which includes providers and natural supports. Collaborate with members of the client’s team to ensure quality of treatment and coordination of care
  • Serve as an advocate for clients, linking them to needed community services; assess client’s personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course
  • Provide strengths-based, client-centered, and trauma-informed clinical services to children and adolescents with mental health diagnoses and co-occurring substance abuse disorders
  • Maintain documentation and chart in compliance with Medicaid, Commission on Accreditation of Rehabilitation Services (CARF), and agency standards
  • Perform other related duties and/or projects as assigned

Qualifications

 

  • Master’s Degree in Social Work, Psychology, or Counseling required
  • Licensed Clinical Professional Counselor or Licensed Clinical Social Worker in Illinois preferred
  • Minimum two years’ experience working with people with mental health and/or substance abuse diagnoses
  • Experience working with children, adolescents, and/or transition age youth and their families required, preference for experience with ages 6-18.
